The memorial plaque commemorates Vanessa Anderson who died in November 2005 after being given a high dose of painkillers in hospital for a fractured skull two days after she was hit by a golf ball at Asquith Golf Club.
The incident, among others, lead to a 2008 commission authored by Peter Garling, SC, to investigate the standard of patient care in public hospitals, which found a prevalent problem associated with the care of the deteriorating patient.
