The plaque commemorates Scott Tweddle who was killed by a lightning strike while loading his truck near Derby, Western Australia, in 2001. 

Note : This plaque has incorrect date of birth, and date of death for Scott Tweddle.  The correct date of birth was 3rd August 1958, and the correct date of death was 13th January 2001.
