The plaque commemorates Lieutenant Sydney R. Coulter who was killed in action during the South African (Boer) War in 1901. 

Ballarat, Sunday. A memorial service of the late Lieutenant Sydney Coulter, of the 5th Victorian Contingent, who was killed at Klombane Mountains on August 23 (sic), was held on Sunday night at the Cathedral.  The service was attended by the members of the 3rd Battalion Infantry Brigade, of which deceased had been a subaltern. The Dean of Ballarat preached.
